Letter of request
The letter of request is a formal letter that asks for something. So the person who writes this letter has an issue and is asking for a change in a situation. Since it is a formal request for something, the issue or problem that you are having must be very clear because you are seeking a favourable response to your request. The explanation for the issue must cover what, where, when, how, who, and, where possible, why. Like the letter of complaint, contact information is also relevant, along with a serious tone, because you have a serious issue that needs to be addressed. Please be polite because you do not want to alienate the person that might solve your problem.
